If you are a veterinarian: Appointments can be scheduled by clients directly when they contact us by phone or email, or on a referral basis through your practice. If you prefer to handle scheduling as a referral, you may do so by completing the form, "Greater Minnesota Veterinary Behavior Services -- Referral Form." To obtain this form you may call us, email us, or simply click here: Referral Form. Simply fax or email the completed form back to us, and we will then contact the client. We ask that a copy of the patient's medical record be faxed or emailed to us as well, prior to the appointment.
